This page is operated by EatSmart by Poonam, the nutrition practice of Poonam Dewan, Clinical Dietitian. [TO BE COMPLETED: registered business name, registered address, and GST or registration number if applicable]

When you submit the form on this page, we collect:

• Your first name — so the messages we send you are addressed to a person.
• Your WhatsApp number — this is the address the guide is delivered to. It is the only field we genuinely cannot do without.
• Your email address — optional. Leave it blank and nothing about the delivery changes.
• Your consent — the fact that you ticked the box, so we can show you agreed to be messaged.
• How you arrived at this page — campaign tags and a click identifier carried in the link you followed (for example from an Instagram post or advertisement), plus which page you submitted from. This tells us which posts are worth making. It does not identify you.

We do not ask for, and this page cannot collect, your medical reports, diagnosis, medication, test results or any other clinical information. If you later choose to share those with Poonam in a consultation, that is a separate relationship covered by her practice's own confidentiality terms, not by this page.

Requesting a guide about hypothyroidism suggests something about your health, and we treat what you give us here with that in mind rather than as an ordinary marketing list. We do not sell it, we do not rent it, we do not trade it, and we do not pass it to anyone beyond the service providers listed below who are needed to actually deliver the guide.

• Send you the guide on WhatsApp.
• Send you follow-up messages on WhatsApp with thyroid-related tips, and invitations to free sessions such as the Thyroid Wellness Masterclass — this is the part you consented to with the checkbox.
• Understand, in aggregate, which posts and advertisements bring people here, so we spend our time on the ones that help.

We do not make any automated decision about you, and nothing you enter here affects any care you receive.

Only the providers needed to run this, each handling your details on our instructions:

• Supabase — the database your submission is stored in.
• AiSensy — the WhatsApp Business platform that delivers the guide and the follow-up messages. Your number and first name reach WhatsApp (Meta) as part of that delivery, which is unavoidable when the delivery channel is WhatsApp.
• Our hosting provider — which serves this page and processes the form submission.

We will also disclose information where we are legally required to, which we hope never happens.

We keep your details for as long as you are happy to hear from us, and delete them when you ask. If you have not engaged with anything we send for an extended period we will remove you rather than keep messaging. Reply STOP to any WhatsApp message we send and we will stop. You do not need to give a reason, and it does not affect anything else. You keep the guide.

Under India's Digital Personal Data Protection Act, 2023, you can ask us to tell you what we hold about you, correct anything that is wrong, delete it, or withdraw the consent you gave — as easily as you gave it. Write to REPLACE-ME@eatsmartbypoonam.com and we will act on it.

This page stores the campaign tags from the link you followed in your browser's session storage, so they are still available if you move between pages before submitting. They are cleared when you close the tab, and they identify a campaign, not a person.
